Request for Disclosure of Personal Data and Record of Provision of Personal Data to Third Parties

Requests for “Notification of the Purpose of Use of Personal Data”, “Disclosure of Personal Data”, “Correction, Addition or Deletion of Personal Data”, “Suspension of Use or Erasure of Personal Data”, or “Cessation of Provision of Personal Data to Third Parties” from the person or his/her representative with respect to the Personal Data retained by us must be made in writing by following the procedures described below.

1.Required documents

(1)Request Forms

We will send you a form by mail upon request by contacting our Personal Data Inquiry Desk. You can also download and print your copy from the following links. Please be sure to select the appropriate form for your request.

("Deletion" here means partial deletion of Personal Data.
e.g., You have provided your address, name and phone number to SECOM, but the address is incorrect, and you want to delete it.)

("Erasure" means the complete erasure of Personal Data retained by us.)

(2)Identification Documents

[For requests by the person himself/herself]

  • Original certificate of residence

and one of the following documents or certificates:

  • Copy of driver's license
  • Copy of passport
  • Copy of health insurance card
  • Original certificate of seal registration
  • Copy of alien registration certificate
  • Copy of pension handbook

[For requests by a representative]

The above identification documents for both the person and the representative

  • Original certificate of residence

and one of the following documents or certificates:

  • Copy of driver's license
  • Copy of passport
  • Copy of health insurance card
  • Original certificate of seal registration
  • Copy of alien registration certificate
  • Copy of pension handbook

and;

  • Letter of proxy from the person(Stamped with the person’s own personal seal)
  • Original certificate of seal registration of the person

(3)Handling Fee

A handling fee of 880 yen (including tax) will be charged for each "Request for Notification of Purpose of Use" and "Request for Disclosure" in addition to the cost of postage. Please enclose postage stamps equivalent to the handling fee.

Please send your request by "acceptance-recorded mail" or "simple registered mail" to ensure that it reaches us.

Postage is the responsibility of the requester.

3.Response to your request

We will verify your identity before sending you a response. The identification documents you have sent us will be returned when we respond to you.
Please contact us, should you wish to receive an electronic record of the information subject to your request for disclosure.

4.Cases in which we are unable to respond to your requests

Please note that we may not be able to respond to your requests such as for disclosure in the following cases or for legal reasons.

  • If the documents and fees you have submitted are incomplete or insufficient.
  • In case the person's identity cannot be verified.
  • When the right of representation cannot be confirmed for a request by a representative.
  • When the subject of the request does not fall into the category of "Retained Personal Data" as defined in the Act on the Protection of Personal Information, or when SECOM is not obliged to respond to the request by law.
